一、 美國(guó)fmc無(wú)船承運(yùn)人在中國(guó)交通部備案

1、 申請(qǐng)書(shū)（格式由代理提供）；

2、 可行性分析報(bào)告（格式由代理提供）；
feasibility analysis report (samples to be supplied by the agent)
3、 企業(yè)商業(yè)登記文件，須由企業(yè)登記機(jī)關(guān)確認(rèn)或證明該復(fù)印件與原件一致的公證文書(shū)原件，需大使館認(rèn)證；
business registration document of enterprise, with copies notarized to be fitted with the original and verified by prc’s embassy; notarization and verification must be original.
4、 提單原格式樣本（背面條款要清楚，要使用carrier，不能用freight forwarder）
original sample of bill of lading (items must be clear on the reverse side; carrier must be used, but not freight forwarder)
5、 聯(lián)絡(luò)機(jī)構(gòu)說(shuō)明書(shū)，載明聯(lián)絡(luò)機(jī)構(gòu)名稱、住所、聯(lián)系方式及聯(lián)系人、簡(jiǎn)介（中國(guó)簽章）；
deion of the contacted organization, with the organization’s name, office, contact ways, contact person and its general introduction (to be chopped with the chinese office’s common seal)
6、 委托書(shū)（格式由代理提供）；

7、 委托人與聯(lián)絡(luò)機(jī)構(gòu)的協(xié)議書(shū)（格式由代理提供）；
contract between the client and the contacted organization (samples to be supplied by the agent)
8、 聯(lián)絡(luò)機(jī)構(gòu)工商登記檔復(fù)印件（中國(guó)簽章）；
business license of the contact organization (copy to be chopped by the chinese office’s)
9、 法人代表、投資方、1名海運(yùn)部負(fù)責(zé)人簡(jiǎn)歷；
resume of the legal representative, the investor and 1shipping operators;
10、 提供fmc下已交存12.5萬(wàn)美元保證金的證明及美國(guó)nvocc證書(shū)復(fù)印件。
proof of depositing 125,000 usd into fmc and copy of american nvocc certificate.
備注：以上材料一式二份，由國(guó)外公司簽字蓋公章（除提單外，提單一式三份不蓋章）, 用a4紙打印復(fù)印，用黑色水筆簽字，蓋章需清晰完整，所有外文材料需要翻譯成中文，蓋翻譯公司公章并附上翻譯公司工商登記檔，所有格式檔請(qǐng)勿改變格式。
note,references mentioned above must be supplied in duplicate, be chopped with the common seal of foreign company, expect bill of lading; bill of lading must be offered in triplicate without seal. meanwhile, references should be printed or copied with a4 size paper, by gel ink pen of black color; documents should be wholly chopped clearly; all the foreign language references should be translated into chinese, to be chopped with the common seal of the translation institute, plus its business license. all the tables should be supplied in the original type.
